The 2015, Darke County Community Healthy Needs Assessment conducted by Wright State University prioritizes health issues to include 1.) Chronic Disease (cancer, diabetes, heart, and obesity); 2.) Mental Health and Substance Abuse; and 3.) Chronic Lung Disease.

DCGHD administrative staff is responsible for developing and maintaining the operational infrastructure that supports the performance of essential public health functions and effective financial management systems.

The Division Investigates health problems and environmental public health hazards, and enforces public health laws to protect the community. Offers two voluntary programs free of charge to prenatal women, children, and families: 1. HMG Early intervention provides family-centered services for infants and toddlers to age 3 with a developmental delay, disability, or a medical condition likely to result in a delay or disability. 2. HMG Home Visiting provides first time expectant or new parents the information and support they need to be prepared for the birth of their child and provides ongoing education and support for families to maximize their child’s health and development to age 3.

DCGHD Help Me Grow Early Intervention  Do you have a concern about your child’s development? HMG Early Intervention service needs are determined by a multidisciplinary team who evaluates and/or assesses the child’s developmental needs. The team is led by a Service Coordinator who will work to assist and enable a child with a delay or disability and the child’s family to receive services and rights as afforded under the Individuals with Disabilities Education Act. Children and their families will transition when they complete needed services or when the child turns age three, with supports to move on to the next phase of growth & development.

DCGHD Help Me Grow Home Visiting  Home Visiting matches parents with a trained Home Visitor who comes to wherever the family lives. Their Home Visitor is a friendly, trained, and experienced person who can answer questions about pregnancy or being a parent and can help find services that help families care for themselves and their children. The HMG Home Visiting Program works with and supports families and helps them set and reach their goals. In home services include: parenting education, ongoing screenings, coordination and referrals for community resources, transition at age 3. Public health nursing is the practice of promoting and protecting the health of populations using knowledge from nursing, as well as social and public health sciences. The DCGHD Nursing Division provides a wide variety of public health services to the community including vaccination of children and adults against disease, surveillance of disease, helping families with children who have medical handicaps, education on sexually transmitted diseases, assisting teen and first time moms, providing health education, and conducting home health visits. Darke County Public Health is constantly exploring new ways to promote and protect the public’s health in Darke County.  New programs and opportunities are either in planning stages or are currently being implemented in order to fill gaps in the community’s well being.  While some of these programs don’t easily fit into defined categories, they remain just as important. Health Education-Coming Soon; Public Health Data Informatics-Coming Soon; Prescription Drug Drop Box-The Ohio Attorney General’s Office partnered with the Ohio Department of Health and the Drug Free Action Alliance to provide free collection bins to local law enforcement agencies in Ohio. The drop boxes are mailbox-style disposal bins that can be placed inside law enforcement agencies to be used by residents during normal business hours. Darke County Prescription Drug Drop Box Location: City of Greenville Police Department, 122 West Main Street, Greenville, Ohio. 937.548.1103. Pills can be taken out of bottle and placed in a plastic bag. Accepting pill form only, no liquids; and Public Health Inspection Reports-Coming Soon.

Wayne HealthCare is a rural, nonprofit acute care hospital located in Greenville, Ohio. The hospital provides inpatient, outpatient and emergency services to patients in the Darke County community.

Lifestyle Enhancement Center is an outpatient clinic that focuses on prevention and control of chronic diseases. The LEC staff will work with you to make lifestyle and sometimes life saving changes. These changes can help to improve your quality of life and manage your disease. Cardiac Rehabilitation program offers an individualized treatment plan to assist patients with managing symptoms of heart disease, decrease the chance of additional heart attacks, and improve endurance and strength to regain the ability and continue a healthy lifestyle. Cardiac Rehabilitation

Pulmonary Rehabilitation program focuses on improving the quality of life and activity levels for patients with pulmonary problems such as chronic obstructive pulmonary disease (COPD).

Wayne HealthCare began offering Pulmonary Rehabilitation services in the Lifestyle Enhancement Center in February 2013.  As a medical facility, it is important to provide a continuum of care for our patients. Physicians, respiratory therapist, and the medical director collaborate to develop individualized treatment plans. Patient education and support is provided to enable patients to cope, live with, and control the symptoms of chronic lung disease.

Healing Hearts Clinic provides a continuum of care by monitoring your health, providing education on topics such as diet, exercise, tobacco avoidance, maintaining a healthy weight, healthy eating, medications, and frequent collaboration with your physicians.

If you have COPD, diabetes or other heart related conditions, you are encouraged to visit the clinic. Healing Hearts Clinic

Blood Pressure Clinic: High blood pressure is known as the silent killer because it generally does not have any symptoms and goes unrecognized. The goal of the blood pressure clinic is to identify and monitor blood pressure in order to prevent other organ damage from untreated or under-treated high blood pressure. The highly qualified staff is available to answer questions and provide advice.

Tobacco Cessation Programs: Everyone nowadays is aware of the risks of smoking. But that doesn’t mean quitting is easy. Wayne HealthCare is committed to educating and helping those in our community take a step toward healthier living by choosing to quit smoking. Freshstart Tobacco Cessation Program consists of four FREE one-hour sessions. All of the methods and activities contain the most effective elements for success. The single most important element is YOU – and your dedication to fight the addiction to nicotine that makes you want to smoke. Freshstart can be your start to a new life without cigarettes! Diabetes Education Program: Diabetes is a disorder that affects how the carbohydrates that we eat are metabolized, resulting in blood sugar levels that are higher than normal. It is important that diabetes be diagnosed and treated because elevated blood sugar levels can damage vital organs like the heart, kidneys, eyes, nerves, and blood vessels.

Once you have diabetes, it will not go away; however, you can control it through education and lifestyle changes. The American Diabetes Association recommends that individuals with diabetes maintain good blood glucose control to prevent or lessen the effects of long-term complications. It is up to you to control your diabetes by eating right, exercising, and losing excess body weight. You may also need to take medication prescribed by your physician.

Diabetes Education Programs include: Diabetic Group Classes, Individual Session with a Certified Diabetes Educator, Individual Session with a Registered Dietitian, Free Monthly Diabetes Education Programs, and Pre-Diabetes Individual Education and Group Class. Comprehensive Health Network (CHN) provides both skilled and non-skilled care. This gives you the benefit of transitioning between skilled services and non-skilled services and back without ever leaving the agency.

CHN is a Medicare certified home care agency, held to the highest of state and federal standards of patient care. CHN nurses and therapists provide quality skilled services and under their guidance, certified nurse aides are available to provide non-skilled personal care and homemaking services. CHN has been providing home care to the residents of Darke County and the surrounding communities since 1988.
